Required Eligibility Documents
Eligibility Verification Documents
(must be provided to complete application process):
- Student/Participant birth certificate, driver’s license or state ID (proof of age and citizenship)
- Student/Participant social security card
(proof of social security number)
- Prior 6 months pay stubs, or employer statement including net pay for prior 6 months, or award letter for cash public assistance or food stamps, if applicable
(proof of income)
Other Eligibility Documents that may apply:
- If participant is in foster care, bring letter from FCCS
- If participant is pregnant or parenting, bring child’s birth certificate or letter from doctor verifying pregnancy
- If participant is an offender, bring letter from Juvenile Court or Juvenile Justice
